Take a screenshot on your phone or print the Florida Blue Medicare Lounge pass and show it upon your arrival to the lounge. Only one form per party needs to be completed.
Please note, all guests who are planning to visit the Florida Blue Medicare Lounge must also have a valid EPCOT® ticket and reservation for the day of their visit.
